OGE Seeks Public Input on OGE Form 201: Request an Individual’s Ethics Documents

The U.S. Office of Government Ethics (OGE) had begun the renewal process for OGE Form 201: Request an Individual’s Ethics Documents.

The form allows members of the public to request Public Financial Disclosure Reports, Periodic Transaction Reports, Certificates of Divestiture, Ethics Agreements, and Certifications of Ethics Agreement Compliance from OGE.

OGE seeks input from the public on proposed changes to the form, which include updating language for accuracy and providing additional information to inform requesters of retention periods that may impact availability of requested documents.

Specifically, OGE would like to know:

  1. What problems do you have using the form?
  2. Are there sections of the form or instructions that are unclear?
  3. Is there information provided that is confusing?
  4. What additional information would be helpful?

You can read about the proposed and see the draft form in this Federal Register Notice. Comments may be submitted by email to usoge@oge.gov by November 10, 2025 or you may register to attend a public meeting on October 6th at 2:00. All comments must be received by November 10, 2025 .
